Liability Waiver
RELEASE OF LIABILITY: I/We hereby apply for membership at Springfield Tennis Club (the “Club”) for the year above. I/We hereby agree to abide by the Club rules as established by the Executive Committee. I/We acknowledge that my/our participation in Club activities involves certain inherent risks of bodily injury or damage to property and that by signing below and participating in such activities, I/we am/are voluntarily assuming such risks. I/We accordingly hereby release, remise, and forever discharge the Club, its directors, officers or any other person or persons associated with the Club from all claims, suits, damages, or actions that I/we may have as a result of injury to my/our person(s) or damage to my/our property arising from or in any way attributable to my/our participation in any Club activity regardless of how it may have been caused. If registering Member is under the age of 18, I, as his/her parent/guardian agree to the above conditions with respect to release of liability.
By agreeing to this form and paying the applicable membership dues (if applicable) you are forthright a Member of the Springfield Tennis Club and are therefore entitled to all the Membership privileges of the Springfield Tennis Club including but not limited to voting at the Annual General Meeting and any special meeting(s) of the Membership.
If you are a parent/guardian registering a minor for a Junior Only membership, the minor is considered a Junior Non-Voting Member of the Springfield Tennis Club and is entitled to all the Membership privileges of the Springfield Tennis Club but is not allowed to vote at the Annual General Meeting and any special meeting(s) of the Membership. Parents/guardians registering a minor for a Junior Only membership are considered Registrant Members and are not entitled to vote at the Annual General Meeting and any special meetings of the Membership.
If you provide an email address and telephone number, you are granting permission for the Springfield Tennis Club to contact you. For more information on being a Member of the Springfield Tennis Club please contact: Edward Li, Membership Director or visit our website at: www.springfieldtennis.ca. You can withdraw your Membership at any time by notifying Edward Li, Membership Director in writing.
Your personal information will not be shared with other Springfield Tennis Club Members unless permission is obtained except for Leagues. Members that are signed up for a League will have their name and phone number shared with other members playing in that same league to ensure proper running of the league.
I/We agree to abide by the Springfield Tennis Club Code of Conduct. The Code of Conduct can be found at www.springfieldtennis.ca or posted at the clubhouse.
I/We agree to follow all COVID-19 rules and guidelines as set by the Springfield Tennis Club and the City of Mississauga.
I/We agree to NOT provide combination entry code number to non-members and acknowledge that the penalty will result in termination of my/our membership
